Shared Care Record - Connecting health and care information

The Hertfordshire and West Essex Shared Care Record connects GP practices, hospitals, hospices, out-of-hours/community/mental health services and adult social care information for health professionals.

Each service providing your care - such as a GP, hospital or community service - holds records separately. The Shared Care Record securely joins up information from these different systems, so staff can view a summary of your health and social care records in one place.

The Shared Care Record is not a portal for individuals to access their own records, but it does mean people delivering care have instant access to the information they need at the right time. 

Consultant Paediatrician at West Herts Teaching Hospitals NHS Trust Dr Richard Burridge describes how having access to a joined-up record has improved care within the Children’s Emergency Department at the hospital:

The Shared Care Record supports us to treat the children we see quickly and safely. We can instantly view key information such as allergies and medications recorded by the patient’s GP, and we get a much fuller picture of the care they are receiving from other providers."

Other benefits include:

  • Less duplication and fewer delays - for example, tests already carried out at a different hospital are not repeated.
  • Time saved - staff can view information from other services instantly, without needing to email or make a phone call.
  • Ability to plan more co-ordinated and personalised care because a practitioner can see which other services are already in place for an individual.
